A COVID-19 story – Our health advisors are helping to support people every day during the COVID-19 crisis.
We recently supported a 49 year old lady of Indian origin who moved to the UK about 8 years ago. She lives with her husband and three sons all of whom have special needs.
The client was originally referred for Exercise on Referral (EoR) by the GP. She was called by a health advisor towards the beginning of the COVID-19 crisis to cancel her EoR appointment and it then became apparent that the client and her family needed extra support. The client’s husband was in hospital with coronavirus and her son was showing symptoms of coronavirus as well. The client’s family had been in quarantine for about 12 days and there was no social or familial support available to get groceries or medication
The client was provided telephone support by signposting her to food banks in Hounslow and making a referral to the HRCH coronavirus volunteer army for support with a weekly drop of groceries.
One of our health advisors helped the client set goals which included being more active within her home. The client’s son also helped her to access the One You Hounslow website to enable the client and the family to access exercise sessions at home.
A discussion was also had with the local Learning Disabilities health team to find out how best to support the family. It was suggested that it might be helpful to educate the client about the virus and the current situation as well as telephone support through mental health organisations.
The client has been receiving a call from the team on a daily basis since early April to make sure food has been delivered to her.
Our health advisor also liaised with the client’s GP and collated a list of telephone numbers of local organisations and charities that could provide support. The client was encouraged to make the relevant phone calls to get the support she needed.
Direct contact was also made with the Open Kitchen team in Hounslow for support with daily cooked meals. When the Open Kitchen staff realised that this was a request from the NHS they went out of their way to make sure that a daily meal was delivered rather than just one off support.
Our health advisor also informed the wider One You Hounslow team about the client’s situation to arrange support with groceries and medication deliveries.
The client has been very appreciative that a member of our team has spent so much time supporting her.
